These are the forms and questionnaires I have used for 25+ years in my Tomatis® centre in Sweden and now in New Zealand for Initial Assessments in person and remote client work. They complement the TED questionnaires or can be used alone depending on your client's profile.
My forms and questionnaires aim more towards learning outcomes and less towards medical or therapeutic applications of the Tomatis® Method. This means that they are well-suited for teachers. They are also great for remote work. If you are a therapist, you no doubt have access to additional tools specific to your field of expertise.
The questionnaires can be used without prior knowledge by discussing the results with your clients or prospects. If you want to reap the full benefit from using them, I suggest you invest in the courses where I share the best way to collect, present, and explain the data collected to your clients. Timing, visual graphics, and explaining contra intuitive data are keywords here.

PS Note: These forms are NOT suitable for severe developmental delays in children with a (developmental age) below five years. Well, it's somewhat useful, but what you need then are the SEAM™ questionnaires and the Foolproof Results course. There I walk you through how to use SEAM™ in detail. It's an advanced course that takes you through test criteria, statistics, professional Excel templates, and child development in a structured "how to" and practical way for your practice. It's not enough to get the SEAM questionnaires. You must also learn HOW to use them, which I will share with you.
